UGC License and Consent

By submitting consent on typeform, I confirm that I am the creator and owner of the photo(s)/video(s) I am uploading and that I have the right to grant this permission. I give ButcherBox For Pets (“the Company”) a perpetual, worldwide, royalty-free, irrevocable, transferable, and sublicensable license to use, copy, reproduce, publish, distribute, edit, modify, and display this content (including any images, audio, video, text, or likeness of me or my pet) in all media now known or later developed, for any commercial or marketing purpose.

I grant the Company permission to use my name, likeness, voice, and my pet’s likeness in connection with the content. If any minor appears in the content, I am their parent or legal guardian and grant permission on their behalf.

I represent and warrant that the content is truthful, does not infringe on any third-party rights, and depicts the product being used safely. I release and discharge the Company from any claims and waive any right to royalties or compensation related to the use of this content.
